Tailored support to access services that enhance wellbeing and quality of life for older people.

The Connecting Seniors and Carers Program offers a soft entry point into the service system, connecting older people and their unpaid carers to a ‘Connector’ that can help them navigate the formal and informal supports available to them.

It’s a flexible, wraparound service designed to strengthen links in local community, support older people to age in place and enhance wellbeing and quality of life.

What type of supports are available through the program?

The program assigns you a dedicated ‘Connector’ staff member, who will support you in finding services tailored to your unique needs.

These may include, but are not limited to:

  • Essential services
  • Community engagement services
  • Family and relationship support services
  • Safety and protection services
  • Legal services
  • Financial wellbeing services
  • Intergenerational activities
  • Education, skills and training
